Forms are available to the public for use during the estate processing. Each form is made available in Microsoft Word and Adobe Acrobat formats.
Instructions -

- Select ‘Save’ and specify the file location to save the forms.
- Unzip the zip file after the download to access contents.

| Inventory Forms & Instructions - Used to provide an inventory of the assets of the decedent. This should be filed with the Register of Wills by the personal representative. | Oath or Affirmation of Personal Representative in Lieu of Filing Inventory and Rendering Final Account - Used by attorneys to be filed with Register of Wills to close an estate where there are no assets. This form is commonly reffered to as a "No Asset Affidavit" and is generally used where an estate was opened solely for litigation purposes and there were no assets and no monetary award to the estate was received from litigation.
